In exchange for being allowed to participate in any class, event, or activity organized by Pop Up Sculpt ("the Organizer"), I agree to the following:

1. Assumption of Risk. I understand that sculpt, Pilates-style, and other fitness classes are physical activities that carry inherent risks, including but not limited to muscle strains, sprains, falls, and in rare cases serious injury. Classes may be held outdoors or in non-traditional spaces with uneven surfaces, weather exposure, and other environmental conditions. I voluntarily accept and assume all such risks.

2. Medical Readiness. I confirm that I am physically able to participate, and that I have consulted a physician about any condition that could be affected by exercise. I agree to work at my own level, modify or skip movements as needed, and stop if I feel pain, dizziness, or discomfort.

3. Release of Liability. To the fullest extent permitted by law, I release and hold harmless Pop Up Sculpt, its organizers, instructors, volunteers, and any venue or property owner hosting a class, from any and all claims, demands, or causes of action arising out of my participation, including those arising from ordinary negligence. This release does not apply to gross negligence or willful misconduct.

4. Personal Property. I am responsible for my own belongings before, during, and after class.

5. Photo and Media. Classes may be photographed or filmed. I consent to the use of images or video that may include me in Pop Up Sculpt's social media and marketing. If I prefer not to appear, I will tell the organizer before class starts and they will accommodate me.

6. Age. I confirm that I am 18 years of age or older.

7. Cancellation and Refund Policy. Reservations canceled at least 24 hours before the scheduled class are eligible for a full refund. For cancellations made within 24 hours of class, a refund will be issued only if the canceled mat is successfully filled by another guest from the waitlist. No-shows are non-refundable. Reservations are not transferable to another person.

8. Waitlist Policy. Once a Pop Up Sculpt drop reaches capacity, a waitlist may be opened. Available mats will be offered to waitlisted guests in signup order. If offered a mat, the guest will receive a private reservation link and will have two hours to complete the reservation before the opportunity may be offered to the next guest. The waitlist closes two hours before the scheduled class. Joining the waitlist does not guarantee a reservation.

9. Event Changes and Cancellations. Pop Up Sculpt may make reasonable changes to an event's location, timing, or format due to weather, venue conditions, safety concerns, or circumstances outside of its control. If Pop Up Sculpt cancels an event and does not provide a replacement option, affected guests will be eligible for a full refund.

10. Agreement and Electronic Signature. By entering my full legal name, checking the required waiver-agreement box, and submitting this form, I confirm that I have read and understand this Participation Waiver, voluntarily agree to its terms, and intend my electronic submission to serve as my signature.
